Job Description

Job Overview We are seeking a compassionate and motivated new graduate Licensed Practical Nurse (LPN) working under a Limited Permit to join our Skilled Rehabilitation team. This position is designed for recent nursing graduates who are awaiting licensure and are committed to obtaining their LPN license within 90 days of hire. Comprehensive supervision, mentorship, and clinical support will be provided. Responsibilities Provide direct resident care under the supervision of a Registered Nurse and in accordance with state regulations. Monitor residents' health status and report changes promptly. Administer medications and treatments as permitted under a Limited Permit. Assist with admissions, transfers, and discharges. Maintain accurate and timely clinical documentation. Collaborate with the interdisciplinary team to support resident care plans. Promote resident safety, comfort, dignity, and quality of life. Follow infection control, safety, and facility policies and procedures. Qualifications Graduate of an accredited Practical Nursing Program. Possession of a New York State Limited Permit or eligibility to obtain one. Must obtain New York State LPN licensure within 90 days of hire. Strong communication, organizational, and interpersonal skills. License Requirement:
