Job Description

Clinical/Technical Responsibilities Assist in scheduling appointments and testing according to protocol. Assist in contacting insurance companies to obtain appropriate insurance authorizations for all testing, and medications as needed. Works closely with patients to communicate any changes and/or concerns. Schedule/track appointments according to protocol and insurance needs. Inform provider of any patient or clinician concerns while in process of scheduling patient for testing (e.g., hospital pre-testing issues, medical clearance issues, etc). Assists patient in the achievement of identified goals by carrying out the prescribed plan of care. Demonstrates patient advocacy by assuring a patient's right to privacy, safety and confidentiality. Maintains current knowledge and skills by participating in relevant educational and training programs. Ability to flexibly respond to changing demands. Conducts pre-visit preparations. Meets environmental responsibilities as assigned which may include setting up equipment according to department protocols; cleaning exam/procedure rooms, and equipment between patient visits to maintain infection control; ordering, sorting, storing supplies; and restocking exam/procedure rooms. Fulfills clinical responsibilities, which include checking vital signs; physical examination preparations; prescription verifications with physician's orders; biohazard waste disposal and monitoring; laboratory procedures including Occupational Safety and Health Administration (OSHA) guidelines; quality control methods; EKG testing; completing lab draws; emergency first aid. Monitor medication, and laboratory supplies, coordinating with CMH hospital departments as appropriate Monitor medical supplies, DME equipment, medication and laboratory supplies, coordinating with CMH hospital departments as appropriate Clinic Stakeholder Responsibilities Participates effectively as a member of the healthcare team. Treats patients, families, visitors and co-workers with courtesy and respect. Provides direct constructive feedback to colleagues as appropriate. Clinic Financial Responsibilities Demonstrates effective time management skills Participates in performance improvement activities by identifying opportunities for improvement, offering solutions and supporting changes. Uses resources efficiently. Utilizes Community Memorial Hospital's property and supplies appropriately and prudently to maximize cost effectiveness of procedures, conserve resources and minimize waste Reviews e-bills and captures all services performed Clinic Health Information Management Responsibilities Offers and receives feedback on own performance and on the quality of patient care delivered. Uses feedback to improve practice and patient care. Consistently ensures that patient information is kept private and confidential.
Job Requirements:
Graduation from an accredited Practical Nursing program is required. Previous out patient experience preferred Knowledge of policies and procedures related to infection control, environmental safety, pain management, emergency situations, fire safety, performance improvement and patient confidentiality. Knowledge of Electronic Medical chart, Meaningful use reports as well as Performance Improvement reports. Skill in appropriate assessment and assistance techniques as applicable. Skill in appropriate use of universal precautions, safe workplace and confidentiality methods. Skill in health information management by appropriately charting patient data. Current NYS licensure as a Licensed Practical Nurse is required. Current BLS certification is required.
