Registered Nurse / Nurse Supervisor Position Summary The Licensed Practical Nurse (LPN) provides direct nursing care to residents in accordance with established care plans, physician orders, and regulatory requirements. Working under the supervision of a Registered Nurse, this role supports the delivery of high-quality, person-centered care while promoting resident safety, comfort, and well-being. Essential Duties and Responsibilities Administer medications and treatments as prescribed by a physician and in accordance with facility protocols. Monitor, assess, and document resident health status, including vital signs and changes in condition. Assist in the development, implementation, and evaluation of resident care plans. Collaborate with the interdisciplinary team to ensure coordinated and comprehensive care. Provide direct nursing care, including wound care, infection control, and other clinical interventions as appropriate. Maintain accurate, timely documentation in the electronic medical record system. Communicate effectively with residents, families, and members of the healthcare team. Respond appropriately to changes in resident condition and emergencies. Ensure compliance with all federal, state, and local regulations, as well as organizational policies and procedures. Participate in required training, in-service education, and staff meetings. Qualifications Current Licensed Practical Nurse (LPN) license in the State of New York. Current CPR/BLS certification required. Ability to communicate effectively and work collaboratively within a team environment. Strong organizational skills and attention to detail. Demonstrated ability to provide compassionate, resident-centered care. Basic computer skills required for documentation. Prior experience in a skilled nursing or long-term care setting preferred, but not required. Working Conditions Frequent standing, walking, bending, and lifting. Exposure to healthcare environments, including residents with acute and chronic conditions. Use of standard medical equipment and personal protective equipment (PPE).
