As a Nurse-LPN at Phoenix House , you will monitor ongoing/chronic medical problems, handle minor needs of patients including attending to minor injuries, administering routine medications, EKG and performs QA activities. You will have responsibilities including but not limited to: On Site daily Support residents in the (self) administration of medication Documentation of (self) administration of medication Performs TB and Drug screenings on all patients within 72 hours of admissions Performs routine urine drug screens as directed by the treatment/recovery plan Monitors and records vital signs Provides triage and provides medical services accordingly For patients with health care needs, schedules and coordinates all medical appointments, and provides periodic follow-up Reviews and transcribes medication orders and monitors medication records for compliance. Observes and records response to treatment/medication and notifies the Medical Director, the RN Supervisor, and the Program Director of any such instances.

Graduate from an accredited nursing programLICENSES/CERTIFICATIONS
Current NYS Licensed Practical Nurse Current certification in BLS and/orACLS REQUIRED EXPERIENCE
At least three years related experience Qualified by training, education, and experience in providing nursing care for chemically addicted patients and patients withdrawing from alcohol and or drugsCOMPETENCIES
