We are seeking a dedicated and clinically skilled Licensed Practical Nurse (LPN) to join our team and provide Certified Home Health Aide (CHHA) services throughout the Westchester region. Position Summary The Licensed Practical Nurse (LPN) will provide CHHA services to clients receiving home care services under the supervision of a Registered Nurse, in accordance with New York State regulations. This role involves delivering clinical treatments, monitoring patient conditions, maintaining documentation, and supporting the overall plan of care. Responsibilities Perform wound care treatments and monitor wound healing progress Administer insulin injections Observe, assess, and report changes in patient condition to the supervising Registered Nurse and physician as appropriate Maintain accurate and timely clinical documentation Ensure compliance with all agency policies, procedures, and regulatory requirements Provide patient and caregiver education within the scope of practice Collaborate with the care team to support positive patient outcomes Qualifications Current and active New York State Licensed Practical Nurse (LPN) license Minimum of one (1) year of nursing experience preferred Strong clinical and documentation skills Excellent communication and organizational abilities Ability to work independently and manage a flexible schedule Reliable transportation and valid driver's license Schedule One week of paid training on agency policies, procedures, and documentation requirements Flexible scheduling available Consistent morning and evening visits available once caseload is established Opportunities for work seven days per week based on availability and client needs Compensation $30.00 per hour , depending on the type of service provided and experience Flexible scheduling and opportunities to build a consistent caseload What We Offer Flexible work schedule Supportive and collaborative team environment Meaningful work serving clients in their communities Opportunity to build a consistent and stable caseload Established agency with over 40 years of trusted service If you are a compassionate and dependable LPN looking to make a difference in the lives of seniors and individuals requiring home care services, we encourage you to apply and become part of the Golden Care LHCSA team.
