DAYS Educational Requirements:

Graduate from accredited school of nursing with current licensure and registration minimally meeting the New York State requirements of a Licensed Practical Nurse. Demonstrates knowledge and understanding of the New York State Practice Act, American Nurses Association (ANA) Standards of Nursing Practice and American Association of Office Nurses Standards of Care.

Professional Requirements:

Must possess incentive and initiative to maintain and update professional knowledge and skill necessary to render optional patient care.

Position Accountable To:

Patient, Nursing Coordinator, Director of the Family Care Centers, Provider, Office Manager, VP/Practice management & Support Services and the VP/Patient & Clinical Services.

Position Summary:

Within the scope of practice as defined by licensure, provides nursing care under the direction and supervision of Provider, Director of the Family Care centers/Nurse Coordinator. Provides quality patient care to the community, continuously working within mission statement of Columbia Memorial Hospital.

Salary Range:
