$33.00 per hour
Schedule:
Monday-Friday | 9:00 AM - 5:00 PM No nights or weekends This role is open exclusively to New York State-Licensed Practical Nurses (LPNs) .
Job Overview:
We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join a team providing community-based mental health services in Ulster County, NY. This role supports adults with serious mental illness through a collaborative, mobile care model. This is a great opportunity for LPNs looking for a weekday schedule, meaningful work, and a team-focused environment.
LPN Responsibilities:
Provide outreach and nursing support to clients in the community Administer and monitor medications in coordination with a Psychiatrist Assist with mental and physical health assessments Coordinate referrals and follow up on care plans Maintain accurate and timely clinical documentation Participate in team meetings and interdisciplinary care planning
Qualifications & Requirements:
