LPN Responsibilities Include:
Provide skilled nursing care to residents Monitor resident conditions and communicate changes to the interdisciplinary care team Administer medications and treatments in accordance with physician orders Maintain accurate and timely documentation Support residents and families with compassion and professionalism Collaborate with CNAs, nursing staff, physicians, and other departments to ensure quality care
Qualifications:
Current New York State LPN License Prior LPN experience in a skilled nursing facility, rehabilitation or similar setting preferred Strong clinical assessment and critical thinking skills; compassionate, patient-centered approach to care Excellent verbal and written communication skills; detail-oriented with the ability to recognize and respond to changes in condition Team-focused mindset with a commitment to collaboration and quality care Fully vaccinated against
