Location:
North Syracuse, NY Job Summary "Navitas Healthcare, LLC" is seeking an experienced Licensed Practical Nurse (LPN/LVN) for a long-term care assignment in North Syracuse, NY. The LPN will provide direct patient care, administer medications, monitor patient conditions, document care activities, and collaborate with the interdisciplinary healthcare team to ensure high-quality resident care. The ideal candidate will have experience in long-term care, rehabilitation, or skilled nursing settings and be comfortable working evening shifts.

PM Hours Per Week:
40 Required Qualifications Active LPN/LVN License Long Term Care (LTC) experience preferred Strong patient assessment and documentation skills Excellent communication and teamwork abilities Ability to work independently in a fast-paced environment Responsibilities Provide direct nursing care to residents. Administer medications and treatments as prescribed. Monitor and document patient conditions and changes. Assist with admissions, discharges, and transfers. Collaborate with physicians, RNs, and other healthcare staff. Maintain accurate patient records and documentation. Ensure compliance with facility policies and patient safety standards.
