Job Description
QUALIFICATIONS
1. Graduate of an approved program for practical nursing 2. Currently licensed in the State of New York as a practical nurse or holds a work permit as required by the New York State Department of Education Division of Nursing 3. BLS CertificationPHYSICAL REQUIREMENTS
1. Ability to stand on your feet for long periods of time 2. Ability to bend and stoop several times each hour 3. Able to lift and move 40 pounds 4. Capable of tolerating a demanding and stressful environmentMAJOR TASKS, DUTIES AND RESPONSIBILITIES
1. Proficient with assessing patient vital signs accurately (blood pressure, pulse rate, respiratory rate and SP02) 2. Accurately relay patient information to the provider or other healthcare professionals such as patient symptoms, reactions, problem, changes in condition, medication information and other medical information. 3. Ability to work together with the provider and patient to relay medical information such as test results, provider recommendations or other medical information. 4. Ensures patients are scheduled for follow up appointments, physicals, referrals or other appointments necessary for the care of the patient. 5. Take an active role in ensuring quality metric benchmarks are met for patients through pre-visit planning, per quality reports and during patient visits. Works collaboratively with the provider to ensure appropriate testing has been ordered to meet quality metrics. 6. Provides patient, family member or legal guardian with appropriate education, instructions or care plan recommendations. 7. Ensures that all orders for the patient are carried our in a timely manner. 8. Administers medications to the patient ensuring the right patient (two patient identifier), right medication, right dose and right route have been verified. 9. Administers medications within the LPN's scope of practice in a safe manner and documents all medications administered. 10. Assists with orienting new staff to OMP along with providing performance evaluations of new orientee. 11. Documents all patient care in a timely and accurate way. Ensures patient's medications, histories and allergies are verified and updated at every visit. 12. Maintains a positive and caring attitude to patients, families, co-workers and others while working at OMP. 13. Maintains a non-judgmental, professional, respectful and courteous attitude while working at OMP. 14. Performs duties willingly and shows initiative within the office that you are working in. 15. Remains calm under stressful situations. 16. Ensures a high standard for hygiene and professional appearance while working at OMP 17. Ensures effective communication with all patients, families and co-workers at OMP. 18. Attends educational in services and other educational programs held at OMP including monthly competency completion. 19. Adheres to OMP's established guidelines and policies. 20. Adheres to a smoke free environment while on OMP campuses. 21. Collaborates with the business office to ensure accurate and timely billing of services delivered at OMP. 22. Ensures strict adherence to HIPAA, infection control practices, OSHA standards and safety practices. 23. Displays exceptional organizational skills and attention to detail. 24. Proficient with computer documentation and other electronic devices required to perform the role of LPN at OMP 25. Ensures accurate documentation of the patients care in OMP's electronic medical record. 26. Floats to other offices when needed at the discretion of the Director of Nursing. 27. Ensures any tests that require a prior auth has one in place prior to scheduling testing. 28. Ensures other tasks as designated by the provider, registered nurse or director of nursing are completed as assigned. - Must be a licensed practical nurse (LPN)
- Previous experience in a clinical setting preferred
- Familiarity with basic nursing procedures and medical terminology
- Knowledge of aseptic technique and sterile processing
