Job Summary Shift - 19:00 - 7:00 (Nights) We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. In this role, you will provide essential nursing care to patients across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Perform comprehensive patient assessments by monitoring vital signs and evaluating physiological status Administer medications and injections accurately following prescribed protocols Assist with medical procedures such as catheterization, tube feeding, and sterile dressing changes Document patient information meticulously using EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems Support discharge planning and coordinate care transitions for optimal patient outcomes Provide specialized care for patients with Alzheimer's disease, memory impairments, or behavioral health needs Manage acute pain and participate in pain management strategies tailored to individual patients Conduct triage assessments in urgent care or emergency settings to prioritize patient needs Collaborate with interdisciplinary teams to develop and implement personalized care plans Educate patients and families on health management, medication use, and post-discharge instructions Maintain compliance with HIPAA regulations and infection control standards
