Schedule:
40 Hours/Week (5×8-Hour Shifts) Available Shifts Day/Evening Variable Shift Day/Night Variable Shift Evening Shift Night Shift Position Overview We are seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to provide high-quality care to adult and geriatric residents in a supportive long-term care setting. The LPN will assist with resident care, monitor patient conditions, support rehabilitation activities, maintain a safe environment, and collaborate with the healthcare team to ensure exceptional patient outcomes. Key Responsibilities Provide direct resident care and assistance with activities of daily living (ADLs) Monitor and report changes in residents' physical, emotional, and psychosocial status Support nutrition, hydration, mobility, transfers, and rehabilitation programs Maintain resident safety and respond promptly to call lights and alarms Accurately document care and observations according to facility protocols Promote resident dignity, comfort, and quality of life Qualifications Active LPN License BLS Certification (AHA) required High School Diploma or GED Experience working with adult and geriatric populations preferred
