Port Jefferson, N.Y Location Fast-paced and fast growing NYS Licensed Home Healthcare Infusion Agency, looking for a Scheduling Coordinator to work in our Port Jefferson, NY office location. This candidate should possess previous scheduling experience, experience working with specialty pharmacies in home infusion therapies, sharp skills, is energetic, motivated, outgoing, and confident to get the job done. We staff national pharmacies with infusion nurses that specialize in the administration of high risk intravenous medications to chronically ill patients in their homes in Suffolk & Nassau Counties, the 5 boroughs of NY, CT, NJ and PA. Tri-State Area ⸻ About Us Critical Healthcare Registered Nursing Services, P.C. is a licensed home healthcare agency specializing in infusion therapy. We partner with specialty pharmacies and coordinate skilled nursing services for therapies including IVIG, antibiotics, biologics, and other complex infusions. With a network of 130+ infusion nurses, we are growing rapidly and seeking an experienced Home Infusion Coordinator to support our expanding operations. ⸻ Position Summary We are seeking a highly organized, experienced Home Infusion Coordinator to work in-office. This is a fast-paced, high-volume role requiring prior home infusion coordination experience. The ideal candidate understands infusion workflows and can manage scheduling, documentation, and pharmacy communication with precision and urgency. This is NOT an entry-level position. Home infusion experience is required. ⸻ Key Responsibilities Please do not apply if you do not have at least 1-year home care coordination experience!! Our potential Candidate must have the following qualifications. Coordinate new infusion referrals from specialty pharmacies Schedule nurses for IVIG, antibiotics, biologics, and specialty infusions Communicate with patients, nurses, and pharmacy partners Track SOCs, reauthorizations, and ongoing therapy schedules Ensure all required documentation is complete and compliant Manage urgent scheduling changes and same-day coverage needs Maintain accurate EMR and internal tracking systems Provide real-time updates to pharmacies and leadership Support regulatory and compliance standards Demonstrates ability to effectively manage multi-line phones and multi-task. Attention to detail and the ability to work well under pressure is essential. Possess excellent customer service skills. Strong organizational skills. Excellent customer service skills Ability to communicate and work effectively with patients and co-workers. Highly motivated, flexible, and able to multitask who works well on a team. Energetic, positive, and self-motivated. Must have computer skills. (Microsoft Office) (Excel) (AlayaCare) Must be able to multi-task in a high paced environment. Must have 1 year or more of homecare coordination experience. Must be available to work On call as needed. ⸻ Qualifications Minimum 1+ years of home infusion coordination experience (required) Strong understanding of infusion therapy workflow Experience working directly with specialty pharmacies Knowledge of IVIG, ABX, biologics, central lines preferred Excellent organizational and multitasking skills Ability to manage high call/email volume EMR proficiency required Strong follow-through and problem-solving skills Available for on-Call hours ⸻ What We Offer Competitive compensation (based on experience) Stable, growing agency Structured systems and leadership support Opportunity for long-term growth
