EVENING SHIFT
Job Summary We are seeking dedicated and compassionate Licensed Practical Nurses (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality resident care . where your expertise will support resident rehab recovery, comfort, and well-being through comprehensive assessments, medication administration, and personalized care plans. This position offers an exciting opportunity to make a meaningful difference in residents' lives while working in a fast-paced, collaborative environment. Duties Administer medications and injections accurately following physician orders. Assist with procedures such as catheterization, tube feeding, venipuncture, and sterile dressing changes using aseptic techniques. Document all patient interactions within Electronic Health Record (EHR) systems. Provide wound care management and support infection control protocols to promote healing and prevent complications. Collaborate with interdisciplinary teams—including physicians, physical therapists, social workers, and case managers—to coordinate comprehensive patient care plans. Support discharge planning by educating patients and families on medication management, post-acute care procedures, and safety measures at home or in assisted living facilities. Experience Valid licensure as a Licensed Practical Nurse (LPN) with current state certification.. Knowledge of medical terminology, anatomy, physiology and pharmacology principles. Experience with specialized patient populations including geriatrics, dementia or Alzheimer's care is preferred.
Pay:
$28.00 - $31.00 per hour
