Graduate of a New York State accredited Nursing Program (RN or LPN) RN or LPN License required

CPR/AED/BLS

certificate required 1-2 years of OPWDD experience preferred, but not required.

Responsibilities:

Monitor health needs to ensure well-being of consumers in our care. Administer daily medications as per policy and procedure. Provide care for minor illnesses, emergencies or injuries to consumers and staff. Maintain communication with consumers and families, guardians and/or residential programs regarding consumer health needs. Participate in the planning of consumer program goals and interventions and maintain paperwork as per policy and procedure. Assist in updating and completing quarterly medication reviews. In conjunction with the RN, assist in the implementation and testing/evaluation to determine a consumer's ability to self-administer medications. Maintain all necessary medical records, notes and documentation as required. Join our Team and Make a Difference!
