101 Adirondack Dr., Suite 3, Ticonderoga, New York 12883 Responsibilities What You'll Do (LPN- Licensed Practical Nurse) Administer medications and treatments safely and accurately (following the "5 rights") Lead and support CNAs in delivering high-quality resident care Monitor resident conditions and report changes to nursing leadership Follow and implement care plans to ensure consistent, individualized care Maintain accurate clinical documentation (MARs, treatment records, nursing notes) Ensure safe handling of medications, including controlled substances From Up to Qualifications What We're Looking For (LPN•Licensed Practical Nurse) Current LPN (Licensed Practical Nurse) or RN (Registered Nurse) license in New York State Experience in long-term care, skilled nursing, or geriatric settings preferred Strong ability to lead and support CNAs and care teams Clear communication skills — both verbal and written A compassionate, respectful approach to resident care Ability to work in a fast-paced, hands-on environment Commitment to delivering care aligned with high clinical and regulatory standards EOE Statement
