Description of job: We are seeking experienced Licensed Practical Nurses (LPNs) to provide patient-centered care within a respected healthcare system serving Veterans in the Hudson Valley region. This opportunity offers stable full-time scheduling and the ability to work alongside an interdisciplinary healthcare team in a variety of clinical settings. Key Responsibilities Provide direct patient care to adult, geriatric, medical, psychiatric, and long-term care patients. Administer medications and treatments in accordance with physician orders and facility policies. Monitor patient conditions and report changes to the RN or provider. Document patient care accurately within the electronic medical record. Collaborate with physicians, nurses, and interdisciplinary team members to ensure quality patient outcomes. Maintain compliance with all facility, state, and federal regulations. Required Qualifications Graduate of an accredited Practical Nursing program. Current, unrestricted New York LPN license. Current American Heart Association BLS/CPR certification. Minimum clinical experience in acute care, long-term care, behavioral health, or related settings preferred. Strong communication and documentation skills. Ability to pass credentialing, background screening, and facility onboarding requirements. Compliance & Credentialing Requirements Successful completion of VA credentialing and privileging process prior to start Credentialing for VA is lengthy and can take up to 60 days Current BLS certification required CPR certification through an American Heart Association vendor required Completion of VA-required onboarding and mandatory training prior to first day of work, including:
