Job Description
LEAP Head Start is seeking a part time LPN to work with children and families enrolled in our program in Washington County. Our centers are located in Hudson Falls, Granville, Whitehall and Cambridge. The Child Health Nurse will schedule days to travel to each center in order to help support our children and families. Agency vehicle provided. As the L.E.A.P. Part Time Child Health Nurse, LPN you will: 1. Work closely with all program staff to provide comprehensive services to children and families. 2. Assist in the review of all health records to assess the health and immunization status of enrolled children. 3. Conduct health screenings and follow up treatment as needed. 4. Respond to and assist with referrals from staff and parents regarding health concerns of children and/or their families. 5. Assist education staff in planning and implementing health activities in the classroom. 6. Maintain an effective recordkeeping system that ensures thorough documentation on all health services provided. 7. Assist the Nutrition Manager in developing and implementing resources and a training program for staff and parents. 8. Serve as an advocate for family and child health care. 9. Assist in recruitment and in the annual revision of the Comprehensive Program Plan, Policies and Procedures, Community Assessment, and in the program self-assessment.
