Job Description

Ashland Village Assisted Living in Ashland, OH is seeking a compassionate and qualified candidate to join our team as a Licensed Practical Nurse (LPN). As an LPN in our Assisted Living community, you play a vital role in supporting residents' health, safety, and overall well-being. You will provide hands-on nursing care while promoting independence, dignity, and quality of life. Working collaboratively with caregivers, leadership, and outside healthcare providers, you will help ensure our residents receive personalized care in a warm, supportive environment. Administer prescribed medications and treatments as ordered by physicians. Monitor residents' health status, including vital signs, and report changes in condition as appropriate. Supervise and support caregivers/medication aides, ensuring safe and effective resident care. Assist with resident assessments, care plan development, and ongoing evaluations. Communicate effectively with residents, families, physicians, and interdisciplinary team members. Maintain accurate and timely documentation related to medications, treatments, and resident observations. Promote residents' independence, comfort, and emotional well-being. Assist with coordination of care, including physician visits and transitions as needed. Perform other duties as assigned to support exceptional resident care. What You Need Active unencumbered certification as a Licensed Practical Nurse (LPN) in Ohio. Experience in assisted living, long-term care, or senior care preferred. Excellent communication and interpersonal skills. Highly organized and detail oriented. Ability to work independently or as part of a team.
