Our work is centered on building caregiver capability and reducing burden over time , through proactive support, consistency, and systems that make care sustainable. Job Description We are seeking an experienced Licensed Practical Nurse (LPN) to join our Service team and support Ohio's Structured Family Caregiving (SFC) program. In this field-based, contract-to-hire role , you will conduct required in-person home visits (both initial and recurring), fulfill licensure-based compliance responsibilities, and work closely with Care Coaches and Registered Nurses to ensure caregivers receive safe, consistent, and compliant support. This role is foundational to Givers' Ohio operations. Success depends on reliable execution, strong judgment, documentation rigor, and comfort working independently within a structured service model . This is an incredibly opportunity to help shape how Givers operates in new markets while making a direct impact on caregivers' lives. Responsibilities Conduct in-person Initial Home Visits (IHVs) upon caregiver enrollment, including relationship-building, review of program expectations, and completion of baseline compliance requirements. Conduct bi-monthly Recurring Home Visits to monitor caregiver well-being, verify ongoing compliance, and identify emerging risks or needs. Manage a regional visit schedule independently , consistently meeting visit timeliness SLAs and on-time completion expectations. Complete visit documentation accurately and on time in accordance with Ohio Medicaid and Givers program standards, including clinical review, sign-off, and escalation support as required. Communicate clearly and consistently with Care Coaches, Registered Nurses, and internal partners to support continuity of care. Flag risks, non-compliance, or concerning patterns promptly and escalate appropriately. Contribute feedback on visit workflows, scheduling practices, and documentation processes to support quality and efficiency as the program scales. Practice within Ohio LPN scope under the direction of an RN or authorized provider; adhere to
OAC 4723
standards. Exercise sound clinical judgment, knowing when to resolve issues independently and when to escalate to RN or Care Coach partners. Support additional duties or projects as required as the Ohio program grows. Who thrives in this role This role is a strong fit for LPNs who: Are comfortable working independently in the field with clear expectations Take pride in timeliness, follow-through, and documentation quality Are attentive to compliance and detail without losing sight of caregiver experience Prefer owning a defined region and building familiarity over time Requirements Active Ohio Licensed Practical Nurse (LPN) license (required). 2-4+ years of experience in home health, community health, or Medicaid waiver programs required; experience with OhioHCBS / SFC
strongly preferred. Strong interpersonal and communication skills; ability to build trust quickly with caregivers in their homes. High comfort with documentation, compliance requirements, and structured workflows. Ability to work independently, manage a field-based schedule, and coordinate closely with a remote team. Proficiency with case management systems, mobile applications, and digital documentation tools. Valid driver's license and reliable transportation for in-home visits. Compassionate, grounded, and professional in sensitive family situations. Clear Ohio BCI background check; FBI check if applicable. Current CPR/BLS, TB/health screening, HIPAA training (or willingness to complete upon hire). Compensation & Logistics (Contract Phase)Hourly Rate:
$34 per hour (inclusive of scheduling, visit activities, and documentation). Expected volume of ~10-20 visits/month during rampMileage Reimbursement:
IRS standard rate (currently $0.725/mile for 2026). Primary coverage aligned to Ohio Area Agency on Aging (AAA) regions , with initial focus on AAA3 & AAA4, corresponding to West Central and Northwest Ohio, including a mix of mid-sized metro (Toledo/Lucas County) and rural counties across Allen, Auglaize, Hardin, Mercer, Putnam, Van Wert, Defiance, Fulton, Henry, Ottawa, Paulding, Williams, and Wood. Coverage assignments are designed to maintain regional density and minimize long-distance travelSchedule :

Upon reaching sustained visit volumes (~40-50/month) and excellent performance, this role will be assessed to convert to full-time with salary and benefitsPay:
