You'll work respectfully alongside our Direct Support Professionals (DSPs) to provide comprehensive healthcare that honors the dignity and unique needs of every resident.

Key Responsibilities Direct Patient Care:

Provide bedside care, administer medications, and monitor vital signs to ensure resident safety and comfort.

Team Collaboration:

Partner with DSPs and LPN/RNs to implement care and respond to medical or behavioral needs.

Health Documentation:

Accurately maintain medical records and document services to track resident progress.

Resident Advocacy:

Serve as a vital link between residents, families, and the broader clinical team. Qualifications Active Ohio Licensed Practical Nurse (LPN) license. Ability to remain calm under pressure and handle complex medical needs with empathy. Commitment to working as part of a multi-disciplinary team to deliver the best possible care. PHYSICAL REQUIREMENTS

This position requires the ability to perform medium work, including exerting 25-50 lbs of force occasionally and 10-25 lbs of force frequently. The role involves frequent standing and walking, occasional sitting, and the ability to push/pull up to 50 lbs (e.g., equipment/carts). Frequent use of the hands for grasping/handling and fine finger dexterity is required. The work may involve awkward postures. Position also requires adequate near and far vision, depth perception, and hearing to safely perform job duties.
