Our Memory Care team supports residents living with Alzheimer's and other dementias through evidence-based practices and a warm, structured environment. Position Summary We're seeking a compassionate, detail-oriented LPN to provide high-quality nursing care to residents in our Memory Care neighborhood during day shift (9:00 AM - 5:30 PM). The LPN will collaborate closely with caregivers, and families to promote safety, comfort, and meaningful daily engagement. Key Responsibilities Provide direct nursing care, including assessments, medication administration, treatments, and documentation per state and facility policies. Monitor resident conditions, recognize changes, and escalate concerns promptly to the DON. Implement individualized care plans and participate in care conferences. Support residents with dementia using validation, redirection, and de-escalation technique. Communicate clearly with residents, families, and interdisciplinary team members. Maintain accurate EMR documentation, medication records, and incident reports. Uphold infection prevention, fall prevention, and safety protocols. Guide and support care staff; help ensure appropriate staffing handoffs at shift change.

Minimum Eligibility Requirements:

Holds a current state LPN license. Previous experience working with the elderly in a healthcare setting preferred. Previous supervisory experience preferred. Ability to establish effective relationships with residents, family members and staff. Strong documentation skills. Ability to represent the facility in a positive and professional manner. Looking for 24-36 hours a week, every other weekend and holiday rotation.
