  • Per Completed Visit Per Visit
  • $42 Downtime (pre-approved wait time)
  • $29/hr "Completed" = visit done in the home + documentation finished in EMR within 24 hours + orders/signatures uploaded + meets QA standards.
Service AreaSchedule Cleveland / Northeast Ohio | Flexible scheduling available. What You'll Do Provide skilled nursing care to home health patients in accordance with the established plan of care Monitor and document patient symptoms, vital signs, and changes in condition Administer medications, wound care, and treatments as directed by the RN Case Manager or physician Communicate effectively with patients, families, and the interdisciplinary care team Provide patient and family education related to care, medications, and disease management Maintain accurate and timely documentation in the EMR within required timeframes What Sets VNA Apart AI-assisted documentation

Requirements Must-Have:
Active LPN license (Ohio) + current BLS certification Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area Minimum 1 year of clinical nursing experience Preferred /
Nice-to-Have:
Prior home health experience Familiarity with HCHB Experience with wound care, IV therapy, or other complex skilled nursing procedures
