POSITION SUMMARY
The Community Health Nurse - LPN delivers skilled, compassionate nursing care to patients in their homes across the Greater Cincinnati area. Working under the direction and supervision of a Registered Nurse, this field-based clinician completes daily home visits to perform focused assessments, skilled wound care, and medication management while reinforcing each patient's plan of care. The role is central to keeping patients safe, stable, and well cared for at home — and to the relationships that define Dioun Healthcare in the communities we serve.
ESSENTIAL RESPONSIBILITIES
Clinical Assessment & Care Complete focused, ongoing assessments at each visit — vital signs, pain, skin and wound status, functional ability, and home-safety screening — documenting clinical data that informs the plan of care. Report findings, changes in condition, and patient concerns promptly to the supervising RN, contributing observations that support RN-led care planning. Carry out physician orders and the established plan of care, delivering skilled nursing interventions within the LPN scope of practice. Wound Care Perform wound care per physician orders and protocol — dressing changes, wound measurement and staging support, irrigation, and negative pressure wound therapy as trained. Track wound progression, document accurately, and educate patients and caregivers on wound management, infection prevention, and warning signs. Medication Management Administer oral, topical, subcutaneous, and intramuscular medications, and provide injections per orders. Support medication reconciliation, observe for therapeutic effects and adverse reactions, and report concerns to the RN and provider. Teach patients and caregivers about medication regimens, safe storage, and adherence. Daily Visits & Coordination Complete scheduled visits across the assigned territory each day, managing time and routing to meet patient needs and visit frequency. Communicate with the interdisciplinary team, families, and providers to ensure continuity and timely follow-up. Complete accurate, timely visit documentation in the electronic health record (WellSky) in accordance with regulatory, payer, and agency standards.
QUALIFICATIONS
Required Current, unencumbered Ohio LPN license in good standing with the Ohio Board of Nursing. Graduate of an accredited practical nursing program. Current
CPR / BLS
certification. Minimum of one (1) year of nursing experience. Valid Ohio driver's license, reliable transportation, and current auto insurance. Ability to pass background check, drug screen, and required health screenings. IV therapy certification, or willingness to obtain it, consistent with Ohio LPN scope. Preferred Prior home health, hospice, or home care experience. Demonstrated wound care experience or certification. Familiarity with WellSky or a comparable home health EHR. Exposure to OASIS-based home health documentation and workflows.
PHYSICAL & WORK REQUIREMENTS
Daily local travel to patient homes within the Greater Cincinnati territory. Ability to lift, transfer, and reposition patients and to assist with mobility as needed. Ability to bend, stoop, kneel, and stand for extended periods during care delivery. Work in a variety of home environments and adapt to changing conditions.
COMPENSATION & BENEFITS
Annual compensation of $58,000 - $67,000, commensurate with experience, credentials, and visit productivity. Mileage reimbursement for business travel within the assigned territory. Eligibility for medical, dental, and vision coverage. Paid time off and access to a 401(k) retirement plan.
