In this vital role, you will deliver compassionate, comprehensive nursing services directly in patients' homes, promoting health, safety, and independence. This position offers an energizing opportunity to make a meaningful difference in the lives of individuals across diverse communities. You will utilize your clinical expertise to assess patient needs, develop personalized care plans, and coordinate services that improve quality of life. Duties Conduct thorough patient assessments, including vital signs, physical examinations, and detailed health histories to inform tailored care plans. Administer medications, injections, IV infusions, and other treatments following strict protocols and safety standards. Manage complex wound care, including dressing changes and aseptic techniques for sterile procedures. Provide specialized care for patients with conditions such as Alzheimer's disease, dementia, developmental disabilities, or those requiring hospice or palliative support. Educate patients and family members on disease management, medication administration, nutrition, and lifestyle modifications to foster independence. Utilize electronic health record (EHR) systems like Epic or Cerner to document patient interactions accurately and efficiently. Coordinate with multidisciplinary teams for discharge planning, case management, and referrals to community resources or specialty services such as cardiology or pulmonology. Experience Valid RN or LPN license in good standing with the state licensing board. Prior experience in home health care is highly preferred; experience in acute care settings such as ICU, emergency room, or hospital medicine is a strong plus. Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth. Knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and medical documentation standards aligned with HIPAA regulations. Hands-on experience with IV insertion, catheterization, venipuncture, phlebotomy, and sterile techniques required for safe patient care. Background working with diverse populations including pediatrics (neonatal ventilator management), geriatrics (memory care), behavioral health patients, and individuals with disabilities. Critical thinking skills demonstrated through triage responsibilities and urgent care scenarios; ability to perform physical assessments and diagnostic evaluations confidently. Strong understanding of physiology principles related to respiratory management (ventilators), cardiac care (cardiac catheterization), infection control practices, and pain management strategies.
