Full-time (40 hours/week) Position Summary EverHeart Hospice is seeking a compassionate and patient-focused Licensed Practical Nurse (LPN) to support our Palliative Care team. This role provides clinical support, patient education, care coordination, and follow-up services for patients facing serious or life-limiting illnesses. The LPN works closely with providers, patients, families, and referral sources to ensure quality care and continuity of services. Responsibilities Provide nursing care under the direction of the RN and/or provider. Monitor and report changes in patient condition and symptoms. Educate patients and caregivers on treatment plans and goals of care. Coordinate follow-up visits, referrals, labs, and appointments. Maintain accurate EMR documentation. Assist with admissions, pre-admission planning, and patient follow-up. Collaborate with physicians, nurse practitioners, referral sources, and interdisciplinary team members. Support patients and families through the Palliative Care and In Touch programs. Assist with medication orders, DME coordination, and other clinical support duties. Minimum 3 years of nursing experience in hospice, oncology, medical/surgical, or community health. Palliative care experience preferred.
