Job Description
Job Overview Sapphire Rehabilitation is seeking a compassionate and skilled Licensed Nurses (LPN/RN) to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients in various settings, including hospitals, nursing homes, assisted living facilities, and home care environments. The LPN will work under the supervision of registered nurses and physicians, ensuring that patients receive the best possible care while maintaining their dignity and comfort. Responsibilities Administer medications and treatments as prescribed by healthcare providers. Perform patient assessments, including monitoring vital signs and documenting medical histories. Assist with daily living activities such as bathing, dressing, and feeding patients. Provide wound care and assist with catheterization and tube feeding. Support patients with Alzheimer's care, dementia care, and memory care needs. Collaborate with interdisciplinary teams to develop and implement patient care plans. Maintain accurate medical documentation in compliance with HIPAA regulations. Educate patients and families on health management and disease prevention. Participate in discharge planning and case management for ongoing patient support. Utilize EMR systems such as Epic, Cerner, or eClinicalWorks for patient records. Experience Candidates must possess a valid LPN license with experience in one or more of the following areas: Critical care experience or ICU experience in a Level I or Level II trauma center. Long-term care experience in nursing homes or assisted living facilities. Hospital experience in emergency medicine, acute care, or hospital medicine. Proficiency in IV infusion techniques, medication administration, phlebotomy, and basic life support. Familiarity with medical terminology, ICD coding (ICD-9/ICD-10), and vital sign monitoring. Familiar with Point Click Care (PCC)
