Founded in 1967, The HealthCare Connection was Ohio's first Federally Qualified Health Center (FQHC). Our mission is to provide quality and accessible primary healthcare services through community responsive approaches that address financial, geographic, and other barriers to care for residents of northern Hamilton County and surrounding areas. Paid Holidays Position Summary:
We are currently seeking a Licensed Practical Nurse (LPN) for our Forest Park Health Center location. This full-time role is essential in supporting clinical operations and delivering compassionate, patient-centered care.
Key Responsibilities :
Assist providers with direct patient care and clinical procedures. Conduct patient interviews and accurately document medical information in the Electronic Health Record (EHR). Perform routine screenings and tests as ordered by the provider. Handle telephone inquiries, provide clinical advice within scope, and conduct follow-up as needed. Ensure exam rooms are properly stocked, sanitized, and equipped. Coordinate referrals and pre-authorizations, and communicate next steps to patients. Maintain patient confidentiality and comply with all HIPAA regulations.
Qualifications :
Graduate of an accredited Licensed Practical Nursing (LPN) program Current LPN license in the State of Ohio Active CPR certification
Preferred:
Minimum of one (1) year of LPN experience in a community health, hospital, or private practice setting Strong communication and patient service skills Ability to work effectively in a team-based, fast-paced environment Familiarity with EHR systems is a plus
