LPN (Licensed Practical Nurse)

Licensed Practical Nurse (LPN) Accessible Home Services is currently seeking a caring Licensed Practical Nurse (LPN) to work with clients to provide educational and engaging supports for an adult with intellectual and developmental disabilities.

Pay Rate:

$25 -$27 per hr. (DOE) Licensed Practical Nurse (LPN)

Minimum Requirements:

Current valid and unrestricted practical nursing license Valid Driver's License, proof of insurance At least one (1) year of experience Must be able to pass background check

LPN Essential Functions:

Reporting to RN and Program Coordinator, providing the following nursing and support services (list not all inclusive): Medication Administration G Tube Feeding Assistance Routine wellness checks Collaborate with staff (and assist) in providing direct care supports in accordance with individual ISP's. Maintain clear and detailed written and electronic documentation, following all protocol. Clear and concise communication with RN, individuals, guardians, and other stakeholders in support of continuity of care. Provide direct support for individuals in home setting, as well as in the community Sensitive to client needs, expectations, individual differences and abilities. Treats clients with care, dignity and compassion at all times.

LPN Physical Requirements:

Must be able to stand for long periods of time. Must be able to frequently walk, stand, bend, squat as well as lift, push or pull up to 50 lbs. Must be able to assist clients in maintaining a clean and safe environment, to include assisting with toileting and personal hygiene care. Must be able to respond quickly to clients in a crisis situation in order to ensure the safety and wellbeing of all parties
