Role:
As a Licensed Practical Nurse (LPN) in an assisted living facility, you will play a critical role in providing compassionate and professional nursing care to elderly. Your primary objective will be to support residents' health and well-being by administering medications, monitoring vital signs, and assisting with daily living activities in accordance with established care plans. You will collaborate closely with registered nurses, physicians, and other healthcare professionals to ensure comprehensive and personalized care. Additionally, you will be responsible for maintaining accurate medical records and communicating effectively with residents and their families to address concerns and provide education. Your work will directly contribute to enhancing the quality of life and safety of residents within a supportive and respectful environment.
Minimum Qualifications:
Current and valid Licensed Practical Nurse (LPN) license in the state of employment. High school diploma or equivalent required; completion of an accredited practical nursing program. Basic Life Support (BLS) certification or equivalent CPR certification. Demonstrated knowledge of nursing principles, medication administration, and patient care standards. Ability to pass background checks and meet health screening requirements.
Preferred Qualifications:
Experience working in assisted living, long-term care, or geriatric nursing settings. Familiarity with electronic health record (EHR) systems and documentation software. Strong interpersonal and communication skills to effectively interact with residents, families, and healthcare teams. Continuing education or training in dementia care.
Responsibilities:
Administer prescribed medications and treatments accurately and timely according to physician orders and facility protocols. Monitor and record residents' vital signs, symptoms, and overall health status, reporting any changes or concerns promptly to the supervising nurse or physician. Assist residents with activities of daily living such as bathing, dressing, mobility, and feeding while promoting independence and dignity. Maintain detailed and confidential medical records, documenting care provided, observations, and any incidents in compliance with regulatory standards. Collaborate with interdisciplinary care teams to develop, implement, and update individualized care plans tailored to each resident's needs. Provide emotional support and health education to residents and their families, fostering a positive and trusting care environment. Ensure compliance with all state and federal regulations related to assisted living and nursing care practices.

The required skills enable the LPN to deliver safe and effective nursing care by accurately administering medications, monitoring health indicators, and responding to residents' needs. Strong observational and documentation skills ensure that all care activities are recorded precisely, supporting continuity and quality of care. Communication skills are essential for educating residents and families, as well as collaborating with healthcare professionals to coordinate care plans. Preferred skills such as experience with electronic health records streamline daily documentation and improve information sharing. Additionally, specialized knowledge in geriatric and dementia care enhances the nurse's ability to address the unique challenges faced by assisted living residents, promoting their comfort and well-being.
