Job Summary We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings, including assisted living and acute care environments. The LPN will work under the supervision of registered nurses and physicians, ensuring that patients receive appropriate medical attention and support. Responsibilities Administer medications and treatments as prescribed by healthcare providers. Monitor patients' vital signs and report any significant changes to the supervising nurse. Assist in the development and implementation of patient care plans. Provide personal care to patients, including bathing, dressing, and grooming. Maintain accurate medical records and documentation in compliance with healthcare regulations. Perform catheter care and assist with IV insertion as needed. Support patients with dementia care by providing emotional support and engaging activities. Collaborate with healthcare team members to ensure comprehensive patient care. Educate patients and their families on health management and wellness practices. Skills Strong knowledge of anatomy and physiology to understand patient needs effectively. Proficient in acute pain management techniques to alleviate patient discomfort. Familiarity with Durable Medical Equipment (DME) usage and maintenance. Experience using electronic medical record systems, particularly Epic. Ability to provide compassionate care in assisted living settings. Excellent communication skills for effective interaction with patients, families, and team members. Strong organizational skills to manage multiple tasks efficiently within a fast-paced environment.
