Job Description

Overview Join our dynamic healthcare team as a Licensed Practical Nurse (LPN), where your expertise will directly impact patient care and well-being. In this vital role, you will provide compassionate, efficient nursing services across various settings, including long-term care facilities, hospitals, outpatient clinics, and home health environments. Your skills in patient assessment, medication administration, and clinical procedures will help ensure optimal health outcomes for diverse populations. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and detailed medical histories to inform treatment plans. Administer medications and injections accurately following established protocols, including IV infusion and catheterization procedures. Utilize electronic health record (EHR) systems such as Epic, Cerner, or Athenahealth to document patient information meticulously and ensure compliance with HIPAA regulations. Assist with complex clinical procedures such as wound suturing, spinal taps, or gastrostomy feedings while maintaining aseptic techniques. Monitor patients' conditions closely, managing acute pain and providing triage support in urgent or emergency situations. Collaborate with physicians, case managers, and other healthcare professionals to coordinate discharge planning and post-acute care. Support specialized care needs including Alzheimer's care, dementia management, memory care, hospice support, and working with individuals with developmental disabilities or disabilities. Conduct specimen collection and processing for laboratory testing; perform phlebotomy and venipuncture with precision. Educate patients and their families on health coaching topics such as nutrition, medication management, or chronic disease management. Ensure all medical documentation is accurate and up-to-date in accordance with NCQA standards and ICD coding requirements (ICD-9/ICD-10). Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) with current CPR/BLS certification. Proven experience in diverse healthcare settings such as hospitals (including ICU or Level I/II trauma centers), nursing homes, assisted living facilities, or outpatient clinics. Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, eClinicalWorks, or Athenahealth for documentation and patient management. Strong knowledge of physiology, anatomy, medical terminology, and diagnostic evaluation techniques. Experience in specialized areas such as critical care, emergency medicine, internal medicine, geriatrics, pediatrics (including neonatal ventilator management), or infectious disease care is highly desirable. Ability to perform complex clinical procedures including airway management, tube feeding administration, sterile processing techniques, and use of medical imaging tools when applicable. Excellent communication skills for effective patient assessment and education; compassionate approach to working with vulnerable populations including children and seniors.
