Job Description
InCare Ohio Home Health is hiring for our Medication Management Program! It will be home health visits inside one facility! We are looking for 1 FT LPN and 1 PT LPN Shifts would be 4 to 6 hours for PT which are flexible and FT 6a to 6p Job Summary Join our dynamic home health team as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) specializing in medication management. This vital role involves providing expert nursing care to patients in their homes, ensuring safe and effective medication administration, and supporting overall health and wellness. Working 12-hour shifts, you will be a key player in delivering compassionate, patient-centered care that promotes independence and quality of life. Your expertise will help bridge the gap between clinical needs and home-based support, making a meaningful difference in our patients' lives every day. Responsibilities Administer medications accurately according to physician orders, including injections, IV infusions, and oral medications Conduct comprehensive patient assessments, including vital signs, physical examinations, and medication reconciliation Monitor patient responses to treatments, managing acute pain and adjusting care plans as needed Maintain detailed medical documentation within Electronic Health Record (EHR) systems such as Epic or Cerner, ensuring compliance with HIPAA regulations Educate patients and their families on medication usage, side effects, and safety precautions to promote adherence and understanding Collaborate with interdisciplinary teams on discharge planning, case management, and coordination of services for complex cases such as dementia or Alzheimer's care Perform specialized procedures including catheter care, tube feeding management, venipuncture, phlebotomy, and sterile techniques for infection control Skills Proficiency with EMR/EHR systems for accurate documentation Strong background in diverse settings such as long-term care facilities, assisted living communities, hospitals (including Level I Trauma Centers), nursing homes, hospice care, or outpatient clinics Experience with medical coding systems including ICD-9/10 and CPT coding for precise recordkeeping and billing Skilled in patient assessment techniques across various populations including pediatrics, geriatrics, individuals with disabilities or developmental delays Expertise in infection control practices and aseptic techniques to ensure patient safety during invasive procedures Knowledge of pain management strategies and acute care protocols for managing complex cases involving trauma or infectious diseases Ability to work independently while maintaining excellent communication with healthcare teams to ensure seamless patient care Join us if you're passionate about delivering high-quality home health services that empower patients to live healthier lives.
