Job Description

Overview We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care in various settings, including hospitals, nursing homes, and assisted living facilities. As an LPN, you will play a vital role in supporting the healthcare team and ensuring the well-being of patients through effective nursing practices. Duties The responsibilities of the Licensed Practical Nurse include but are not limited to: Administering medications and treatments as prescribed by physicians Conducting patient assessments and monitoring vital signs Assisting with daily living activities for patients, including those with developmental disabilities and Alzheimer's care Performing basic life support and emergency interventions when necessary Collaborating with healthcare professionals to develop and implement patient care plans Documenting patient information accurately in electronic health record (EHR) systems such as Epic, Cerner, or eClinicalWorks Providing education and support to patients and their families regarding health management and treatment plans Managing IV infusions, catheter care, and tube feeding as required Assisting in triage and emergency situations within various care environments Experience Qualified candidates should have: Previous experience in critical care, long-term care, assisted living, or home care settings Familiarity with medical terminology, ICD coding (ICD-9/ICD-10), and medical documentation practices Proficiency in patient assessment techniques and administering injections, venipuncture, or specimen collection/processing Experience working with diverse populations including geriatrics, pediatrics, and individuals with disabilities or chronic conditions Knowledge of infection control practices and aseptic techniques Ideal candidates will also have experience in areas such as: Acute care settings including emergency medicine or ICU experience Hospice care or palliative services for end-of-life support Health coaching or case management for chronic disease management We welcome applicants who are passionate about nursing and committed to providing exceptional patient-centered care.
