Job Description

Oklahoma City, OK | On-Site Only | RNs & LPNs | MDS & RAPs Experience Required We are seeking a skilled and detail-oriented MDS Coordinator to join our healthcare team. The ideal candidate will have strong knowledge of the Minimum Data Set (MDS), Resident Assessment Protocols (RAPs), clinical documentation, and healthcare regulations. This role offers the opportunity to make a meaningful impact on patient care quality while working in a fast-paced healthcare environment that values accuracy, professionalism, and compassionate care. Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) are encouraged to apply. Day-To-Day Responsibilities Gather and complete MDS data, including quarterly assessments, in compliance with RAI guidelines and HCFA requirements. Review RAPs and related resident information to ensure accuracy and completeness. Maintain organized MDS and Care Plan tracking systems to support timely submissions. Ensure all MDS documentation accurately reflects each resident's current health condition based on assessments and medical records. Coordinate care plan decisions and document all required information according to state and federal regulations. Submit and electronically transmit MDS data in accordance with state and federal reporting requirements. Qualifications Current, active, and unrestricted Oklahoma RN or LPN license. Working knowledge of MDS and RAPs, with the ability to develop individualized and comprehensive resident care plans. Current CPR and First Aid certification. Proficiency with basic computer applications, including Microsoft Office.
