Job Description

Position Summary Provides skilled nursing care to patients in their home under the supervision of a Registered Nurse. Provides service in accordance with the Home Health Agency's philosophy, standards and policies. Position Accountabilities Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards. Provides nursing care and ongoing support in the home as ordered on the plan of treatment and nursing care plan. Functions as a team member demonstrating collaboration with, and responsibility to patient, registered nurse, and physician. Ability to adequately assess and monitor disease processes. Utilizes appropriate disease management techniques. Educates the patient and family regarding disease management. Responds to the physical, mental, social and emotional needs of patients and their families. Demonstrates knowledge of medications and their correct administration based on age of the patient and the patient's clinical position. Follows the five medication rights and reduces the potential for medication errors. Addresses and supports cultural practices, as long as such practices do not harm others. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical health care errors. Adheres to all agency, payroll, billing and documentation policies and procedures. Participates in agency's other mandated activities, as requested by the Home Health Director including in-services and committees. Demonstrates flexibility, organizational skills, and ability to exercise sound judgment. Maintains patient confidentiality at all times. Follows policies and procedures regarding attendance and work hours. Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops. Attends meetings as required. Performs other related duties as assigned or requested.
