Job Description

Licensed Practical Nurse (LPN)Job Summary We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. In this role, you will provide exceptional patient care in a variety of clinical settings, including long-term care facilities, assisted living communities, hospitals, rehabilitation centers, and outpatient clinics. The ideal candidate is committed to delivering high-quality nursing care, promoting patient well-being, and collaborating with interdisciplinary teams to achieve positive health outcomes. Key Responsibilities Provide direct patient care through assessments, monitoring of vital signs, and implementation of individualized care plans. Administer medications, injections, IV therapies, and other prescribed treatments in accordance with established protocols. Maintain accurate and timely patient records using electronic health record (EHR) systems while ensuring HIPAA compliance. Assist with diagnostic testing, specimen collection, and medical procedures to support patient evaluation and treatment. Deliver specialized care to patients with chronic illnesses, memory care needs, hospice and palliative care requirements, or complex medical conditions. Monitor patient conditions, identify changes in health status, and communicate findings to healthcare providers. Support emergency and acute care interventions, including airway management, pain control, and patient stabilization when appropriate. Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure coordinated, patient-centered care. Educate patients and families on treatment plans, medications, and health maintenance strategies. Qualifications Current and unrestricted Licensed Practical Nurse (LPN) license in the state of practice. Current CPR/BLS certification; additional certifications are a plus. Experience in long-term care, rehabilitation, hospital, clinic, or acute care settings preferred. Knowledge of medical terminology, infection control practices, patient assessment techniques, and clinical documentation standards. Proficiency with electronic medical record (EMR/EHR) systems such as Epic, Cerner, eClinicalWorks, or Athenahealth. Experience providing care to diverse patient populations, including geriatric, adult, pediatric, and memory care patients. Strong communication, organizational, and critical-thinking skills. Ability to work effectively in a fast-paced environment while maintaining a high standard of patient care. Why Join Us? $32.60 - $39.85 per hour
