Job Description

The primary focus is to facilitate the transition of a potential patient from the acute care facility or community to the care of home health. The LPN Referral Coordinator begins the referral process at the point of introduction to the patient and records information in the EMR for transfer to the home health agency. The LPN Referral Coordinator will determine if appropriate criteria are met for admission to home health and continue to facilitate the transition to services through the time the patient is scheduled for admission. The position serves as a liaison and will help promote home health within the community for potential referral sources, physicians and other health care providers by building relationships through educational and consultative services. The position assists with quality management by completing review of all OASIS documentation and working with clinicians for the most accurate and complete documentation possible. Quality review of other clinical documentation as needed to maintain compliance and quality standards. The LPN Referral Coordinator will be required to participate in the nursing after hour and weekend on-call schedule, with an RN available as reference. The position will include patient care including responsibility for coordinating and administering the total nursing care of any given home health patient. They may be responsible for executing the patient's plan of care, and for acting as a patient advocate. The nurse will inform the nurse case manager and/or health care provider and other applicable personnel, of changes in the patient's condition and needs, and counsels the patient and family in meeting nursing and related needs. The nurse will be competent in care of patients of any ages, including pediatrics, as needed. The coordinator will serve as field nurse during times of staffing need.
Education:
Graduation from an accredited school of nursing and current Oregon nursing license without restriction required. Current BLS required. Competency with MS Office Suite (Word, Excel, Outlook) and EPIC preferred.
Work experience:
1 year of verifiable work experience as a nurse in an acute care setting required. 1 year of verifiable work experience as a nurse in a community health setting preferred.
Skills & Competencies:
Ability or aptitude for coordination of interdepartmental and nursing communications. Highly efficient, self-motivated organizational and interpersonal skill level, with role model customer service skills. Must have knowledge of current regulatory and clinical practice standards relating to home care setting including recent knowledge/experience with Patient Driven Grouping Models, and Value Based Purchasing. Must have the clinical knowledge and critical thinking ability to effectively coordinate patient care consistent with standards and regulations. Must have strong oral and written communication skills. Able to prepare and maintain patient records and reports. Able to react calmly and effectively in urgent and emergent situations. Ability to multi-task, with excellent time management skills Must establish and maintain positive and effective working relationships with staff, health care providers, patients and facilities and other departments. Flexibility, independence, initiative and motivation required to work competently and autonomously. Must possess the ability to deal tactfully with personnel, patients, family members, visitors, health care providers and general public. Able to establish priorities and an effective workflow to efficiently balance field duties with office duties. Must demonstrate strong personal integrity, stamina and emotional stability.
ESSENTIAL JOB FUNCTIONS
Share responsibility for 24 hour on-call on a rotational basis with other Home Care Services nursing staff (RN available per scope of practice requirements). Have a vehicle in reliable condition with insurance coverage and a current Oregon Driver's license. Ability to lift 50 pounds' maximum with frequent lifting or carrying of objects weighing up to 25 pounds. Ability to drive a vehicle in all types of weather. Ability to withstand extreme seasonal temperatures. Able to complete patient visits efficiently, meeting required productivity standards. Ability to communicate verbally with patients, family, and members of the health care team.
