Position Summary The Hospice LPN provides direct patient care and support under the supervision of a Registered Nurse Case Manager , ensuring comfort, dignity, and quality of life for hospice patients. This role is essential in executing care plans, managing symptoms, and supporting families in both home and facility settings. This is a field-based position requiring travel throughout the Lincoln City and surrounding coastal communities . Key Responsibilities Patient Care Provide skilled nursing care to hospice patients in accordance with the care plan Monitor patient condition and report changes to the RN Case Manager or interdisciplinary team Assist with symptom management, including pain control and comfort measures Administer medications as directed (within LPN scope of practice and state regulations) Care Coordination Collaborate with RN Case Managers, physicians, social workers, chaplains, and aides Participate in interdisciplinary team (IDT) meetings and care discussions Support implementation of individualized care plans Patient & Family Support Educate patients and families on hospice care, medications, and disease progression Provide emotional support during end-of-life care Reinforce care techniques to caregivers to ensure patient comfort Documentation & Compliance Maintain accurate, timely, and compliant clinical documentation Follow all Medicare hospice regulations and Oregon state requirements Ensure proper communication and documentation of patient status changes Quality & Safety Adhere to infection control and safety standards Observe and report any safety concerns in the home environment Support quality initiatives and continuous improvement efforts Qualifications Required Active Licensed Practical Nurse (LPN) license (Oregon or compact eligibility if applicable) Minimum 1 year of nursing experience (hospice, home health, or long-term care preferred) Valid driver's license and reliable transportation (field-based role) Ability to work independently and manage a flexible schedule Preferred Prior hospice or home health experience Strong understanding of end-of-life care principles Experience working in rural or coastal service areas Key Competencies Compassionate, patient-centered care approach Strong communication and interpersonal skills Ability to navigate sensitive end-of-life conversations Excellent time management and organization skills Alignment with CAPLICO values and team culture Work Environment Primarily field-based across homes, assisted living, and skilled nursing facilities Travel throughout Lincoln City and nearby coastal areas Work schedule may include occasional on-call or weekend rotations Compensation & Benefits (Lincoln City, OR Market)
Compensation Hourly Rate:
$28
