JOB SUMMARY
The Memory Care Unit Director (LPN) is responsible for overseeing the clinical operations, resident care services, and day-to-day functioning of the Memory Care neighborhood at Firwood Gardens. This position ensures residents receive compassionate, person-centered care in a safe and supportive environment while maintaining compliance with Oregon Administrative Rules, company policies, and regulatory requirements. The Memory Care Unit Director serves as a clinical leader, mentor, and resource for caregiving staff, collaborating closely with the Executive Director, Regional Nurse Consultant, physicians, families, and interdisciplinary team members to promote exceptional resident outcomes and quality of life.
RESPONSIBILITIES
Coordinate and oversee resident care services within the Memory Care unit. Conduct resident assessments, service plan reviews, and ongoing evaluations. Monitor residents for changes in condition and ensure timely communication with physicians, families, and healthcare providers. Participate in admissions, re-admissions, and resident transitions. Review incident reports, investigate concerns, and implement corrective actions as appropriate. Ensure accurate and timely documentation in accordance with company policies and state regulations. Assist with medication management oversight and delegated nursing responsibilities as permitted under Oregon regulations. Monitor infection prevention and control practices throughout the community. Provide supervision, coaching, and support to caregivers, medication aides, and other direct care staff. Participate in hiring, onboarding, training, and performance management activities. Conduct regular staff education and competency validation. Foster a positive culture focused on resident-centered care, teamwork, and accountability. Assist with scheduling and staffing coordination to ensure adequate coverage. Promote person-centered approaches to dementia care. Support implementation of individualized resident interventions that enhance quality of life and resident engagement. Collaborate with Life Enrichment and other departments to ensure meaningful programming tailored to resident needs. Support behavioral interventions and non-pharmacological approaches to dementia-related behaviors. Ensure compliance with Oregon Assisted Living and Memory Care regulations. Participate in quality assurance and performance improvement initiatives. Maintain readiness for state surveys, inspections, and audits. Ensure resident records, service plans, and clinical documentation remain compliant and current. Monitor and address risk management concerns. Serve as a primary clinical contact for residents and family members. Communicate resident status changes and care concerns in a timely and professional manner. Participate in family meetings and care conferences. Promote a welcoming environment that supports resident dignity and family involvement. Other duties as assigned
REQUIREMENTS
Active Oregon Licensed Practical Nurse (LPN) license. Experience working with older adults, dementia care, memory care, assisted living, or long-term care population preferred. Ability to effectively supervise and support staff. Strong clinical assessment, communication, and organizational skills. Ability to maintain confidentiality and professionalism. Current CPR/First Aid certification or ability to obtain upon hire. Ability to successfully complete required background checks.:
