Job Description

Job Summary We are seeking a dedicated and energetic Licensed Practical Nurse (LPN) / Registered Care Coordinator (RCC) to join our dynamic healthcare team. In this vital role, you will provide compassionate patient care, coordinate clinical services, and support interdisciplinary teams across various healthcare settings. Your expertise will help ensure high-quality care delivery, efficient patient assessments, and effective management of medical documentation. This position offers an exciting opportunity to make a meaningful difference in the lives of diverse patient populations, including those with complex medical needs and disabilities. All roles are compensated, recognizing the importance of your skills and commitment. Key Responsibilities Complete and update resident assessments and individualized service plans per Oregon Administrative Rules Monitor resident health status, respond to changes of condition, and coordinate with providers, pharmacies, and families Support medication management, including oversight of MARs, medication passes, and staff competency Provide leadership, coaching, and real‑time support to caregivers to ensure consistent, high‑quality dementia‑informed care Participate in move‑ins, care conferences, and regulatory compliance activities Maintain accurate, timely documentation and ensure adherence to safety, infection control, and Memory Care program standards Qualifications Current Oregon LPN license in good standing Experience in Memory Care, Assisted Living, or Residential Care preferred Strong assessment, communication, and relationship‑building skills Ability to lead with professionalism, empathy, and teamwork in a fast‑paced care environment
Pay:
$26.00 - $36.18 per hour
