This role includes assessing health needs, developing care plans, coordinating appointments and follow-up care, training staff, and supporting the overall health and safety of program participants. Step by Step, Inc is a non-profit organization that provides supports to over 2000 individuals with intellectual disabilities, autism, mental illness, and substance abuse. Our community-based programs provide a setting where individuals we serve can be independent. Join our dedicated healthcare team as a Nurse-RN/LPN serving the Philco, Delco, and Montco regions! This dynamic role offers an exciting opportunity to provide compassionate, expert nursing care in our residential and non-residential setting Responsibilities Coordinate medical and dental care, appointments, and follow-up treatment Develop and implement healthcare plans and medical protocols Train staff and caregivers on medical procedures and safety practices Monitor participant health and maintain required documentation Support medication management, emergency response, and quality assurance activities Reviews Medical Appointment/Discharge Paperwork and ensure all new orders are in place. Informs the Director of any changes in program participant's health that may negatively impact their well-being. Responsible for completing the HRST (Health Risk Screening Tool) Clinical Review for all individuals in Program Actively supports Program Managers and host families in carrying out medical activities and appointments. Develop protocols as needed for specific program participant home care situations and in conjunction with the program management team. Creates and updates Lifetime Medical Histories and update Medical Listings Inspects and maintains Durable Medical Equipment. Assists with Pre-Licensing Inspections and create POC's based on ODP licensing results. Conducts and documents routine monitoring and assessment of the participant's health and safety Utilize EMR (Electronic Medical Records) systems to document patient care accurately and efficiently Qualifications and Experience Current Pennsylvania RN or LPN license Minimum 5 years of nursing experience Human services experience preferred Valid driver's license Step By Step is proud to be an Equal Opportunity Employer.
