The ideal candidate will provide compassionate resident-centered care while maintaining professionalism, flexibility, and strong clinical documentation skills in a long-term care environment. Candidates must be comfortable working variable shifts, including days, evenings, and nights. Key Responsibilities Provide direct patient care in a long-term care setting Administer medications and treatments as prescribed Monitor resident conditions and accurately document changes Maintain complete and accurate patient records using electronic charting systems Collaborate with interdisciplinary healthcare teams to support quality resident care Assist residents with daily living activities and supportive care needs Ensure compliance with facility policies, safety standards, and infection control procedures Participate in shift reporting and care coordination activities Required Qualifications Active LPN License required Previous Long-Term Care (LTC) experience preferred PA Department of Aging Fingerprint Clearance Letter required Ability to work variable shifts including days, evenings, and nights Strong communication and patient care skills required For more details contact at hdavda@navitashealth.com or Call / Text at 516-862-1169.
