Licensed Practical Nurse (LPN) - Long Term Care / Skilled Nursing Job Overview We are seeking a compassionate and reliable Licensed Practical Nurse (LPN) to join a skilled nursing and long-term care team. This position offers the opportunity to provide high-quality resident care in a collaborative environment focused on patient safety, comfort, and well-being. As scheduled Requirements Active and unrestricted LPN license Previous Long-Term Care, Skilled Nursing, or Rehabilitation experience preferred Ability to work day and/or night shifts as needed Strong clinical assessment and medication administration skills Excellent communication and teamwork abilities Ability to provide compassionate, resident-centered care Current BLS certification preferred Responsibilities Provide direct nursing care to residents in accordance with individualized care plans Administer medications, treatments, and nursing interventions as prescribed Monitor resident conditions and promptly report changes to the healthcare team Complete accurate and timely documentation of patient care activities Assist with admissions, transfers, and discharges Collaborate with physicians, nurses, CNAs, rehabilitation staff, and other healthcare professionals Ensure compliance with infection control practices, facility policies, and regulatory standards Respond appropriately to emergencies and resident needs Promote resident comfort, dignity, safety, and quality of life Facility Highlights Skilled nursing and long-term care setting Specialized memory support services Comprehensive rehabilitation and restorative nursing programs Onsite physician, laboratory, vision, dental, and podiatry services IV therapy and therapeutic diet support Collaborative interdisciplinary care environment Community Highlights Located near outdoor recreation, scenic parks, hiking and biking trails, white-water rafting, and local dining destinations Shopping, restaurants, and additional amenities available within a short drive
