)JOB SUMMARY
Provides all necessary services needed to assist hospice patients to live in the Marquette Hospice House, becoming part of the Hospice team to carry out the patient's plan of care.JOB DUTIES
Provides direct care to guests (hospice patients) who live in the home. Assist in the facilitation of smooth patient transition, greets guests and their families courteously. Prioritizes patient care. Provides all identified needs of our guests under the direct supervision of the Hospice RN who operates under physician orders. Administers medication to guests as regulated by the nurse practice acts and in accordance with guests' plan of care. Manages medication box and medication audits. Maintains the organized flow of nursing information and documentation. Answers all phone calls courteously and relays messages appropriately. Provides assistance with all ADL's. Supports the hospice philosophy. Provides family education while working with the interdisciplinary team. Provides emotional support for our guests and their visitors. Responds to guest and visitor requests and complaints with immediate notification to direct supervisor or designee. Completes daily household chores to ensure home is clean i.e. laundry, dusting, sweeping, dishes, etc. Works cooperatively with the volunteers that will be helping with household chores. Maintains a safe environment for our guests and their visitors. Demonstrates knowledge of emergency procedures. Plans and prepares meals, snacks, and supplements as needed. Maintains close communication with RN supervisor regarding guest status. Notifies RN of any changes in guests' condition in a timely manner. Performs other duties as assigned.
