Job Description

The Middletown Home is seeking a compassionate, talented and caring LPN to join our Skilled Nursing Department on our Post-Acute Care Unit full-time Monday thru Friday 700am-3:00pm. As the LPN you will practice person-directed care, mentor and coach direct caregivers and perform nursing functions which will include delivery of medication and treatments to provide excellent care to our residents. This individual is also involved in our admission and discharge process as well as working closely with our Therapy Team and other disciplines to foster a resident centered atmosphere.
