We are currently seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our Skilled Nursing team. Position Summary The Licensed Practical Nurse (LPN) is responsible for providing direct nursing care to residents in accordance with physician orders, care plans, and facility policies and procedures. This role works closely with the interdisciplinary team to support residents' physical, emotional, and psychosocial well-being while ensuring quality care and resident safety. The ideal candidate is compassionate, detail-oriented, and committed to delivering exceptional resident-centered care in a collaborative Skilled Nursing environment. Key Responsibilities Resident Care & Clinical Support Provide direct nursing care to residents in accordance with individualized care plans Administer medications and treatments as ordered by physicians Monitor resident conditions and promptly report changes to the RN Supervisor or physician Assist with resident admissions, transfers, and discharges Support residents with daily living needs while promoting dignity, comfort, and independence Respond appropriately to resident emergencies and urgent care situations Ensure residents receive quality, compassionate, and timely care Documentation & Compliance Maintain accurate and timely nursing documentation Document medications, treatments, incidents, and resident observations appropriately Ensure compliance with state regulations, HIPAA requirements, and facility policies Follow infection prevention and safety protocols Participate in audits, quality improvement activities, and required in-services Team Collaboration & Communication Communicate effectively with residents, families, physicians, and interdisciplinary care teams Work collaboratively with CNAs, nurses, therapists, and other departments Provide support and guidance to nursing assistants as needed Assist in maintaining a positive and professional work environment Qualifications Required Active Pennsylvania Licensed Practical Nurse (LPN) License Graduate of an accredited Practical Nursing program Strong communication and organizational skills Ability to work effectively in a fast-paced healthcare environment Preferred Previous Skilled Nursing, Long-Term Care, Rehabilitation, or Assisted Living experience Experience with EMAR, PCC, Point of Care, or electronic healthcare documentation systems Experience working with geriatric populations Physical Requirements This position may require: Standing, walking, bending, lifting, and assisting residents throughout the workday Ability to respond quickly in emergency situations Exposure to resident care and healthcare environments Schedule & Work Environment Full-time and Part-time opportunities available Day, Evening, and Overnight shifts available Weekend and holiday rotation may be required Skilled Nursing environment Why Work at Dunwoody Village?
