Job Description

Overview We are seeking a dedicated and energetic Licensed Practical Nurse Assessment Coordinator (LPNAC) to join our dynamic healthcare team. In this vital role, you will coordinate comprehensive patient assessments, facilitate clinical documentation improvement, and support utilization management processes across various care settings. Your expertise will help ensure accurate medical records, compliance with healthcare standards, and optimal patient care delivery. Responsibilities Conduct thorough assessments of patients' clinical status, utilizing your physiology and anatomy knowledge to inform care planning and documentation. Review and analyze medical records, ensuring accuracy and completeness in accordance with HIPAA regulations and medical terminology standards. Collaborate with interdisciplinary teams to facilitate discharge planning, case management, and utilization review activities across inpatient, outpatient, hospice, and home care settings. Utilize EMR (Electronic Medical Record) systems such as Cerner, Epic, Athenahealth, or eClinicalWorks to document findings, update medical records, and support clinical documentation improvement initiatives. Apply CPT coding, ICD-9/10 coding, DRG assignment, and other medical coding practices to ensure precise billing and compliance with Medicare and NCQA standards. Participate in medical management activities including utilization management, case management, and quality assurance audits to optimize patient care pathways. Support clinical staff by reviewing documentation for accuracy, consistency with clinical protocols such as ICU or Level I Trauma Center standards, and adherence to healthcare regulations. Qualifications Valid license as a Practical Nurse (LPN) with current certification as a Licensed Practical Nurse Assessment Coordinator (LPNAC). Proven experience in managed care environments or hospital settings; critical care experience preferred including ICU or Level II trauma center exposure. Strong knowledge of medical terminology, physiology, anatomy, ICD coding (ICD-9/10), CPT coding, DRG assignment, and healthcare documentation standards. Familiarity with EMR/EHR systems such as Cerner, Epic, Athenahealth or eClinicalWorks; experience with Meditech or similar platforms is a plus. Experience in utilization review, discharge planning, case management or clinical documentation improvement programs. Excellent organizational skills with the ability to review complex medical records efficiently while maintaining high attention to detail. Knowledge of HIPAA regulations and health information management best practices for safeguarding patient data.
