As an integral part of our home health services, you will provide comprehensive nursing support, promote health and wellness, and ensure safe, effective treatment plans tailored to each individual's needs. Your dedication will help foster independence and improve quality of life for diverse populations across various settings. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and detailed health histories to inform personalized care plans. Administer medications, injections, IV infusions, and other treatments following strict protocols and HIPAA regulations to ensure patient safety. Manage wound care, catheterizations, tube feedings, and ventilator support with precision and aseptic technique. Collaborate with interdisciplinary teams using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems such as Epic or Cerner to document patient progress accurately. Provide education on disease management, medication adherence, nutrition, and health coaching tailored to individual needs such as dementia care or post-acute recovery. Assist with complex medical procedures including PICU (Pediatric Intensive Care Unit) interventions or hospice care planning when necessary. Support patients with developmental disabilities or those requiring specialized memory or Alzheimer's care through compassionate engagement and case management. Requirements Valid state licensure as a Licensed Practical Nurse (LPN) with current certification in Basic Life Support (BLS). Proven experience in diverse clinical environments such as long-term care facilities, hospitals—including Level I Trauma Centers—and outpatient clinics. Familiarity with medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and medical documentation standards. Hands-on experience with EMR/EHR systems like Epic, Athenahealth, or eClinicalWorks; proficiency in medical imaging or laboratory specimen collection is a plus. Strong knowledge of physiology, anatomy, infection control protocols, and medication administration techniques. Ability to work independently in home settings while maintaining high standards of patient assessment and safety. Excellent communication skills for triage calls, family education, and interdisciplinary collaboration; experience working with children or elderly populations is advantageous.
