History - Who we are:
Since 2014, A to Z Care has been the leader of adult day care in West Philadelphia. Our adult day care center serves primarily seniors with health-related care needs where they meet new friends, engage in activities, participate in field trips, and receive quality nursing care.
Position Summary:
Promotes and restores clients' health by completing the nursing process, collaborates with multidisciplinary team members, provides physical and psychological support to clients. Maintains the integrity of the A to Z Care Adult Day Center, by promoting the mission and core values of the organization while performing job functions. Abide by the organization's policies and procedures, including confidentiality standards.
Essential Job Functions:
Comply with PA code Title 6. Chapter 11. Older Adult Living Centers and appropriate accreditation standards Conduct nursing assessments and evaluations Address clients' healthcare needs and document all services rendered to clients Advise clients and caregivers on the prevention of disease and illnesses Conduct quarterly reviews of clients' health status, including dietary needs Assist with creating semi-annual care plans together with other team members Administer appropriate medications to the clients Assist staff in the area of client ADL's, including mealtime, bathroom and personal care Respond to emergencies Provide other skilled nursing services as needed and which can safely be done in the center
Qualifications:
LPN license in PA Experience working with aging adults and adults with functional impairments First Aid certification
Knowledge and Abilities:
  • Basic nursing knowledge
  • Broad medication knowledge
  • Understanding of aging population and their needs
  • Ability to work with minimal supervision
  • Ability to work well with others in the medical profession
  • Ability to exercise independent judgement and to make quick decisions
  • Ability to deal with emergencies quickly and efficiently
  • Follow strict rules of confidentiality regarding clients, caregivers and others seeking services
  • Be a team player
  • Ability to work with caregivers courteously and diplomatically
  • Willingness to participate in professional growth activities
